It is an exciting time to join Mission Neighborhood Centers, Inc. (MNC)!

MNC is seeking full-time Case Manager, who will provide case management services to at risk youth that are at risk for or have contact with the Juvenile Justice System. Will be responsible for a caseload of up to 12 clients under the intensive case management program and 15-25 advocacy, information and referral cases.  Will provide assessments of youth and develop the appropriate community services plan to reduce the crossover into higher levels of detention. This position will advocate for culturally appropriate resources in court and a community release plan and requires working knowledge of relevant community resources and building partnerships amongst community agencies and a compassionate approach to working with youth of diverse economic, social, ethnic, and linguistic backgrounds.

Essential Functions and Responsibilities

Primary Functions

  • Conduct initial client and parent intake and assessment and develop individual youth service plan.
  • Work with Probation Department, Public Defenders, community based agencies, parents, SFUSD schools, and Youth Guidance Center staff to identify and assess the needs of youth detained at YGC or on probation, with an emphasis on serving the monolingual Spanish speaking population.
  • Identify and address youth case issues and participate in weekly case development meetings.
  • Provide linguistically and culturally appropriate internal and external service referrals and follow-ups based on individual participant and parent needs.
  • Be knowledgeable about survival strategies used by youth that have had police contact to deal with ongoing issues such as abuse, violence, assault; and utilize strategies and programming that is comprehensive, safe, empowering, community and relational focused to address these issues.
  • Provides home visits to link clients and families to services and assess families’ needs for employment, housing, mental health services, legal assistance or any services identified by case development team.
  • Maintains complete confidential case files and completes all required data collection reporting forms.
  • Develop, implement and participate in street outreach at schools and some evenings around the community and programming at Juvenile Hall to gain participants.
  • Assists in planning, coordination, implementation and evaluation of activities throughout program and in collaboration with other youth services programs to ensure effectiveness, proper utilization and contract compliance.
  • Assists with supervision and provide leadership development to interns, youth workers, and volunteers.
  • Design and implement evening activities and curriculum development for participants in accordance to the Work Plan and Mission/Vision of the agency in the following areas: educational enrichment, social and life skills development, anger management, conflict mediation, health education, violence prevention, youth leadership, career exploration/job readiness, parent support groups, life skills and recreational activities.
  • Arrange guest speakers, workshop presenters, and career mentors within the program.
  • Plan and implement experiential learning field trips and activities, including local college tours and career exposure outings and activities for clients on caseload.
  • Acts a liaison with schools, community, public agencies, and parents/guardians of participants.
  • Provide safe passage to youth daily and transportation to youth on all outings.
  • Attend regularly scheduled staff, program team meetings, individual supervision meetings, weekly case development sessions and monthly trainings as required.
  • Participate in the planning and production of Rites of Passages ceremonies for participants.
  • Participate in MNC events including planning and implementation prior to and the day of the event.
  • Knowledge and adherence to MNC’s policies, procedures, objectives, and quality assurance.
  • Must wear agency apparel (i.e. t-shirt, polo, jacket, badge, etc.) to be identified as a staff person during work hours-on and offsite.  
  • Supporting other Youth Service Programs as needed.

Qualifications

Education:  AS Degree with an emphasis in Social Work, Counseling, Psychology, Criminal Justice or related field; must be enrolled in college education working towards an BA degree in Social Work, Counseling, Psychology, Criminal Justice or related field, with a current official transcript on file plus at least 2 years of Coordinating Experience; or AS in a related field with at least 3 years of Coordinating Experience; or 6 years of Coordinating and Direct Service Experience.

Experience:  Required 4+ years’ work experience with high risk youth and young adults (ages 10-25).

Driver’s Requirement:  Current CA Driver’s License, current auto insurance and a clean driving record.

Language(s):  English, Spanish preferred
